Home health care

A Home Care Agency That Cares About Your Wellbeing

Some of the Services we provide includes:

Senior Personal Care Services

Bathing includes bed bathing & shower assist, toileting and incontinence care, grooming, repositioning to prevent bed sores

We Dementia/Alzeimers supervision, parkinson's support, IDD support(behavioral cues, daily living skills), TBI supervision, cueing for ADLS, redirection and calming techniques

We provide comprehensive feeding assitance, including G-tube feeding, pureed diets, alternating solids and liquids, and a full range of adaptive and specialized feeding techniques. Our team is experienced in supporting individuals with IDD, Parkinson's disease, dementia, and other conditions that require modified feeding approaches, we are fully trained to monitor for and recognize signs of aspiration to ensure safe and effective meal time support.

We assist with medication reminders and encourage adherence to physician insructions, set-up assistance, we notify the client or families when refills are needed, we observe changes in behavior or side effects and document what the client reports, we keep medication areas clean, safe and organized, check for expired medication.

Our care team helps with conversation and emotional support, recreational activities and hobbies, memory stimulation activities, safety monitoring for wandering or fall risks, social engagement and outings, we assist with light housekeeping including laundry, linen changes, dusting and sweeping, trash removal and organizing living spaces

We assist with transfer with gait belts, stand by assist, bed to chair transfers, wheelchair support, fall prevention
Doctor appointments, grocery shopping, prescription pick-up, social activities and community engagements
NC Medicaid 1915i & Innovation Waiver

If you live with your family we provide Community Living & Supports & if you live in your own home, we provide supported living (CLS) services to you in guideline with NC Medicaid, we accept and partner with all Managed Care Organizations (MCOs) and (LMEs) in North Carolina, including Vaya Health, Alliance Health, Trillium and Partners

Career planning, job coaching, interview help, ongoing support to keep the employment.

We provide 24 hours comprehensive care, or any hours allocated to individuals, we assist to teach and support with daily living skills (personal care like bathing & eating, home skills such as cooking, laundry, & safety behaviors)

we provide temporary relief to families and primary caregivers, we can provide respite care in home or out of home, overnight care, scheduled or emergency respite

We offer specialized care for Adults, teens and children who have experienced traumatic brain or spinal cord injuries. Our team focuses on physical rehabilitation, pain management, and cognitive support, helping children regain strength and mobility. We work with families to create customized care plans that encourage the child’s recovery and adaptation to new challenges, supporting their long-term development.

CAP-DA & CAP-C

For Adults & children with neurological disorders, such as epilepsy, cerebral palsy, or developmental delays, we provide comprehensive care that includes medication management, physical therapy, and seizure monitoring. Our team works with families to create personalized care plans that address the child’s unique needs, focusing on both medical care and developmental support to help them achieve their full potential

We offer expert care for children with quadriplegia or paraplegia, focusing on improving their quality of life through mobility assistance, respiratory care, and skin integrity management. Our team is trained to prevent complications such as pressure sores and respiratory infections while promoting the child’s independence and supporting their daily activities.

Our care for children with immunological disorders, such as primary immunodeficiencies or autoimmune diseases, includes careful monitoring, medication management, and infection prevention. We work closely with pediatric immunologists to ensure that children receive the care they need to strengthen their immune systems and manage symptoms effectively.

For children with blood disorders such as sickle cell disease, thalassemia, or hemophilia, we provide tailored care that includes regular monitoring, transfusions, and medication management. Our pediatric nurses are trained to manage symptoms like pain crises, bleeding episodes, and fatigue, offering families the support they need to navigate these complex conditions.

For children born prematurely, we provide specialized, high-tech care to support their unique developmental and medical needs. Our team is skilled in respiratory support, feeding assistance, and monitoring for growth and developmental milestones. We work closely with families to ensure that premature infants receive the care necessary to thrive and reach their full potential as they grow.
