Eligibility for 1915(i) Services

1915(i) services are for certain people with NC Medicaid. You may be eligible for 1915(i) services if you meet the following requirements:

1. You have an eligible condition.

1915(i) services are for people who need support with activities at home or in the community with the following conditions:

Serious mental health conditions

Severe substance use disorder

Traumatic brain injury

How to Get 1915(i) Services

1. Call and ask for a 1915(i) assessment

These NC Medicaid health plans offer 1915(i) services. Call the number listed on your health plan ID card (or health plan welcome letter) and ask for a 1915(i) assessment:

2. Complete the assessment

You will be contacted to schedule a 1915(i) assessment. The purpose of this assessment is to see which 1915(i) services you need. The assessment can be done in person or through a telemedicine appointment. This can take place at your home or in a convenient place. It can take up to an hour. You should get services within 90 days. There may be other factors, like staff availability, that determine when services can start. The person who helped you with your assessment can give you updates (this could be your Tailored Care Manager). Or call the Member and Recipient Service Line listed on your health plan ID card.

NC Medicaid 1915(i) services are home and community-based services for serious mental health needs, substance use, intellectual/developmental disabilities or traumatic brain injury. If you have two conditions (dual diagnosis), you may qualify for 1915(i) services for both conditions. It does not matter which one is the primary diagnosis. Select a condition to learn more.

Get help with daily tasks at home, like bathing, dressing, cooking, budgeting and more. A trained worker will help you one-on-one based on your individual needs. Depending on where you live, this support at home is called different names:

If you live with your family, this service is called Community Living and Supports.

If you live in your own home, this service is called Supported Living or Community Living and Supports.

If you live in an out-of-home placement (like a group home), this service is called Residential Supports.

Residential Supports helps you live in a structured care setting of your choice, like group homes or Alternative Family Living settings. Get help with daily tasks, like bathing, dressing, cooking, budgeting and more. A trained worker will help you one-on-one based on your individual needs.

Day Supports are day programs, like workshops and after school programs. You might do things like volunteer, go out in the community or learn job skills.

Community Networking helps you meet people in your community. For example, a trained worker can go with you to volunteer or take a pottery class. Payment for classes and conferences is also included.

Community Living and Supports helps with everyday activities like grocery shopping and banking. A trained worker can go with you and teach you how to do these things on your own.

Crisis Services help prevent institutional placement or hospitalization. For instance, this service could help you if you are at risk of losing your job or home. you when there are situations that are dangerous to you or others. For example, they can help you if you might lose your job or home. The staff is trained and ready to help people with I/DD. Trained staff can help:

Identify the type of help you need

Connect you to agencies or psychologists

Create a crisis plan

People with the NC Innovations waiver will get a Tailored Care Manager to help them navigate services. They can answer questions about the NC Innovations waiver, help find doctors, arrange free rides to appointments and be the main point of contact for all things Medicaid.

Behavioral Health and Intellectual/​Developmental Disabilities Tailored Plans

Behavioral Health and I/DD Tailored Plans are specialized care plans designed to support individuals with significant mental health needs, substance use disorders, and intellectual or developmental disabilities. These plans go beyond standard health coverage by coordinating medical care, behavioral health services, long-term supports, and community-based resources under one integrated system.

Our purpose is to ensure that individuals with complex needs receive personalized, consistent, and high-quality care. Tailored Plans connect members with essential services such as therapy, medication management, crisis intervention, residential support, habilitation services, and day-to-day community living assistance. They also help families navigate the care system, providing guidance, advocacy, and access to programs that improve independence, safety, and overall well-being.

Tailored Plans are a new kind of NC Medicaid Managed Care health plan.
