Community Alternatives Program for Children

The Community Alternatives Program for Children (CAP/C) is a specialized Medicaid waiver program created to support children with complex medical, developmental, or physical needs who require a high level of care—often equivalent to what would be provided in a long-term care facility. Instead of placing a child in an institution, CAP/C allows them to receive the same level of essential support in the safety, comfort, and emotional security of their own home.

Every child deserves to grow, learn, and thrive in a loving family environment. CAP/C makes this possible by providing families with a wide range of supports designed to meet each child’s unique health and daily-living needs. This may include skilled nursing care, personal care assistance, in-home aides, respite services, case management, medical equipment support, and home modifications that create a safer, more comfortable living space.

Who is eligible for the CAP-C program?

Medically fragile and medically complex children who are age 0 through 20 years of age

Is determined to require a level of institutional care under the North Carolina Medicaid State Plan

Need at least one or more CAP/C home-and community-based services based on a reasonable indication of need assessment that must be coordinated by a CAP/C case manager
